You might be living with chronic illness or chronic pain that has changed the shape of your daily life. You might be someone who looks completely fine on the outside while managing something invisible and relentless on the inside. You might be navigating stress that has settled so deeply into your body that rest no longer feels like rest.
Whatever brought you here, you have probably been trying very hard, for a very long time, to feel better. You deserve support from someone who truly understands what you are carrying. That is exactly why I do this work.

My name is Sara Graff, LCSW, and I am a licensed clinical social worker in Florida with over 25 years of counseling experience.
I specialize in working with adults living with chronic illness, chronic pain, and the stress that settles into the body over time.
I know life with chronic conditions, not only through my clinical training, but through my own lived experience. Chronic illness became part of my life too, and that changed everything about how I practice.
I am not here to tell you to think positive, push through, or simply manage your symptoms better. I am here to walk alongside you as you build a genuinely different relationship with your body, your nervous system, and your life.
This work is warm, grounded, and paced with care. It is also purposeful and grounded in real tools, because I believe you deserve more than a space to talk. You deserve something real to hold onto between sessions, and a path forward.
I did not set out to create a protocol. I set out to find a path that would help people living with chronic illness find themselves, not by eliminating the condition, but by no longer being defined by it. I could not find that path, so I built it.
After chronic illness became part of my own life, I found myself navigating the same system my clients navigate. The appointments that left more questions than answers. The treatments that addressed one piece but missed the bigger picture. The exhaustion of explaining yourself to people who were trying to help but did not quite understand what you were actually living with.
What I needed, and what I kept hearing my clients needed, was not another technique. It was a cohesive path. One that addressed the body, the nervous system, the emotional weight, and the practical reality of living with a chronic condition. I knew it needed to come from someone who genuinely understood, from the inside.
So I built it.
The 5 S.T.E.P.S. to Empowered Healing was designed from the inside out, drawing from over 25 years of clinical experience, my own personal journey, and approaches that research has shown to make a real difference: Pain Reprocessing Therapy (PRT), EFT tapping, cognitive behavioral therapy (CBT), and mindfulness.
It is a path we walk together. You are not starting over every time you need the tools. You are coming home.

When we begin this work together, the first thing we do is slow down. You have probably been moving fast for a very long time, pushing through, managing, adjusting, and trying to keep up with a life that keeps asking more of you than you have to give, while your body asks you to stop. Our work together creates a different kind of space, one where you do not have to explain yourself.
Sessions are warm, conversational, and paced with your energy in mind. We follow the 5 STEPS framework together, at a comfortable pace that honors where you are while supporting your path forward.
Between sessions, you will have tools to use in your daily life. Not homework that adds to your list, but practices that gradually become part of how you move through your days. You can use them quietly, on your own, in the middle of real life.
Most clients work with me weekly. We meet virtually through a secure telehealth platform, which means you can do this work from wherever you are most comfortable in Florida. For those who prefer in-person support, we can discuss creating a plan to meet together that works for both of us.
You do not have to have it all figured out before you begin. You just have to be open and ready to take that first step forward together.

I completed my graduate training at Washington University in St. Louis and have spent the majority of my career in Florida. I have worked with adults living with chronic illness, chronic pain, anxiety, stress related conditions, and the unique challenges faced by seniors and caregivers.
I am licensed in Florida (License SW7007) and currently see clients virtually across the state, with limited in-person availability in the greater Tampa Bay area.
When I am not working, you will find me spending time with my family, exploring the Gulf Coast, caring for my senior dogs, and continuing my own journey of learning and healing.
